Mountain Community Health Partnership (MCHP) is seeking a full time Behavioral Health Provider to join our team. The Provider will deliver behavioral health services in collaboration with primary care providers, focusing on underserved, uninsured, and high-risk populations. This role is ideal for someone who thrives in a fast-paced, team-oriented environment and is passionate about community-based care.Key Responsibilities:Provide integrated behavioral health services during or in coordination with primary care visits.Utilize motivational interviewing techniques to assess patient readiness for change and provide brief interventions.Conduct behavioral health screenings, including managing the SBIRT (Screening, Brief Intervention, and Referral to Treatment) program for depression and substance use.Coordinate and facilitate group medical visits for patients with chronic conditions.Review patient records to assess behavioral health history and identify opportunities for health behavior change.Participate in interdisciplinary care team meetings and daily 'huddles' to coordinate patient care.Maintain compliance with HIPAA, infection control, and safety protocols.Qualifications:Education: Master of Social Work (MSW) or equivalent. Valid and current license to practice in NC.Experience: Minimum 2 years of supervised clinical experience, preferably in a primary care setting.Strong clinical assessment, communication, and teamwork skills. Proficiency in Microsoft Office.
