Responsibilities About the Role During visits that may last up to
one hour, our providers assess a member’s current health status,
medical history, medication adherence, social circumstances, and
other risks. This approach provides invaluable insights into a
member’s overall health that are often missed during standard
office visits. The Comprehensive Health Assessment (CHA) enhances
care quality and allows us to address multiple care gaps in a
single visit. Our providers: Conduct assessments for adults and
geriatrics, including medical history, diagnosis, treatment, health
education, physician referrals, case management referrals,
follow-up, and thorough documentation in line with our guidelines.
Administer and order necessary point-of-care and lab tests.
Collaborate with physicians, case managers, social workers, family
members, key caregivers, and other medical personnel as needed.
Work alongside Primary Care Physicians (PCPs) to educate patients
and ensure appropriate follow-up. Provide services at the member’s
residence, whether in their homes or Skilled Nursing Facilities.
Benefits of Working with Us Consistency: While assessments are
conducted from Sunday to Saturday between 8 AM and 7 PM, our
full-time providers typically work Monday to Friday from 8:00 AM to
5:30 PM. Competitive Pay: Enjoy an annual base salary ranging from
$95,500 to $158,400 , determined by market analysis and experience.
Incentive Program: Includes uncapped monthly incentives and
year-end bonuses. Comprehensive Benefits: Mileage reimbursement,

holidays, continuing education time/reimbursement, and employee

Along with extensive onboarding training, you will be assigned a
preceptor, receive personalized feedback, and gain free access to
online continuing education. Additional Licensure: If business
needs arise, we will assist you in obtaining additional state
licensure and credentialing in neighboring states or others you may
wish to visit while conducting health assessments. Qualifications
Skills & Experience That Stand Out Master’s Degree or equivalent
experience with successful completion of NP or PA licensure.
Current NP/PA licensure in the state of practice, including
prescription authority or the ability to obtain it as needed.
NCCPA, AANP, AACN, or ANCC board certification in Adult, Adult
Gerontology Primary Care, Adult Gerontology Acute Care, Family,
Emergency, or Acute Care specialty. Current BLS, ACLS, or CPR
certification. Preferred one year of NP or PA experience; new
graduates are encouraged to apply! Willingness to travel up to 15%
of the time (including overnight travel) to various locations as
required. Valid state driver's license and proof of adequate
automobile insurance for the state of residence. Strong computer
skills and familiarity with health/medical record software.
