Job Description:
Agape Care Group is seeking a compassionate and dedicated
Hospice Registered Nurse to join our team in providing exceptional
end-of-life care to patients throughout the Southeast. Our mission
is to offer comfort, dignity, and peace to our patients and their
families during lifes most significant moments. As a Hospice
Registered Nurse, you will play a critical role in enhancing our
patients' quality of life, utilizing a personalized approach
tailored to each patients unique needs. Provide high-quality
nursing care to hospice patients in various settings, including
homes, assisted living facilities, and nursing homes. Assess
patients' medical, emotional, and social needs and coordinate with
a multidisciplinary team to plan effective and compassionate care.
Administer medications, perform medical treatments and monitor
patients’ vital signs to manage symptoms and pain based on
physician orders and hospice care plans. Educate patients and their
families about end-of-life care, ensuring they understand the
treatment and support available. Maintain clear and accurate
patient documentation as required by law, administrative
regulations, and agency policy. Support the patient's family by
providing emotional comfort and guidance throughout the hospice
journey. Strong clinical skills with demonstrated competency in
palliative care and end-of-life symptom management. Excellent
interpersonal and communication skills to interact effectively with
patients, families, and healthcare team members. Ability to make
independent decisions when circumstances warrant such action.
Comprehensive knowledge of the ethical and legal considerations
related to hospice care. Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Must possess a current, unencumbered,
active license to practice as a Registered Nurse in the state of
operation. Certification in Hospice and Palliative Nursing (CHPN)
preferred.
