What You’ll Do Provide primary care to residents in skilled nursing / long-term care settings. Conduct assessments, diagnostics, and manage acute & chronic conditions. Develop care plans aligned with individual goals & coordinate with facility teams. Complete Medicare recertifications and required regulatory documentation. Mentor clinical staff; lead quality initiatives; implement best practices. Act as liaison between facility leadership and clinical teams for care coordination.
