Our 207 bed, acute care hospital offers a wide range of services and is part of an organization with 6 acute care facilities, over 11,000 team members and 400 volunteers. Our CARF Accredited Inpatient Rehab unit is ranked in the top 10% of Rehabs in the US.# It has 18 beds and works with hip fractures, amputations, Parkinson#s, MS, multi-trauma, pelvic fractures, stroke cases, etc.# Our average patient length of stay is 11/12 days and our unit utilizes latest technology such as Omnicell, Epic, and Mindray (for vitals), allowing our team to provide the best care possible to our patients. The rehabilitation Case Manager is an important part of the rehab interdisciplinary team.# Candidate must be a strong team member who assist in facilitate the best discharge disposition for patient under their care and provides administrative support of other members of the team.# Responsibilities include: Evaluating/assessment of assigned patients to determine discharge planning/continuum of care needs in consultation with the interdisciplinary Team. Documentation of initial assessment within compliance with CARF/hospital regulations. Participate in interdisciplinary team Assumes responsibility for signatures on attendance sheet and completion of interdisciplinary team conference. Provide information on community resources and makes appropriate referrals to meet patients# needs when indicated. Discuss Discharge Planning options with patient/families/interdisciplinary staff. Participate in rehab unit quality improvement programs and staff educational in services Coordinate transportation for patients returning home as needed Complete Medical Necessity Forms for any discharge needs or additional resources. Obtain authorization for, DME, Rehab, etc from Managed Care Insurances. Records activity on patient#s medical record following Social Work Policy/Procedures. Complete all necessary forms to expedite services/discharge and coordinate dissemination of medical information to appropriate continuum of care providers. Maintain caseload statistics and patient satisfaction reporting process Assist in planning for discharge to community/home Address special needs of our diverse population; hearing impaired or physically challenged. Knowledge of age specific competencies. Address spiritual and cultural implications on plan of care. Provide excellent customer service Demonstrate knowledge of community resources Formulating and preforming discharge plans to meet each patients individual needs and goals Working cooperatively with physicians, therapy staff, outreach coordinators and all members of the interdisciplinary team. Assisting rehab manager in departmental operations and other department duties based on department needs Participating in hospital programs and committees as needed Desirable Traits Ability to multi-task in busy environment Detail orientated Requirements LMSW Prefer 2-4 years of experience Familiar with standard concepts, practices, and procedures within a particular field.
